Five (5) Wishes Living Will Workshop
The Five Wishes Living Will was developed by Aging with Dignity. This Living Will lets your family and doctors know who you want to make health care decisions for you when you can't. How comfortable you want to be. How you want people to treat you. What you want your loved ones to know. This workshop will review every part of the Five Wishes Living Will and how it can be used in Washington State. Physicians Orders for Life Sustaining Treatment (POLST) Workshop
The Physician Orders for Life-Sustaining Treatment program is designed to improve the quality of care you will receive at the end of life. It is based on effective communication of your wishes, documentation of medical orders and a promise by health care professionals to honor your wishes. Long Term Care Assessment Workshop
Peaceful Journeys staff members are trained to review Long Term Care plans. This workshop is designed to review the most important component of a comprehensive long term care plan. Learn about the full spectrum of resources available to help older adults be independent for as long as possible. Each participant will receive materials, community resources, and important forms to take home and review.
